Medina County Youth
Service Coordination

The Service Coordination team is a group of representatives from various agencies in the community.  The team develops and maintains a plan to provide collaborative services to the Medina County's youths with multiple problems.

The ultimate goal of Service Coordination is to provide services that maintain the youth within his/her own community.  Youth can be referred to Service Coordination through any of the following team member agencies:

  • Any Medina County Alcohol, Drug Addiction, and Mental Health (ADAMH) Board Agency,
  • Medina County Job and Family Services Department,
  • Medina County Juvenile Court,
  • Medina County Board of Developmental Disabilities,
  • Any Public School System in Medina County

Eligibility criteria for Service Coordination include youth that are currently involved with three or more of the above-named systems. 

Service Coordination cases are monitored to assure that services are being delivered and goals are being met.  Case managers are assigned from the team .
