Assessment Team
 

Senior ICAT is an advocacy program for older adults of Medina County.

Senior ICAT is dedicated to supporting and strengthening families, providing or facilitating family-centered, coordinated, community based, preventative and comprehensive services that identify and respond to the health, human, and social service needs of older adults and their families and the professionals that serve them in Medina County.  

This initiative is funded by the Medina County Family First Council (FY 2009-2010).

Senior ICAT is designed to assist seniors by helping bring together all services and agencies that can help advocate and identify needs to keep seniors functioning at their highest level in their home or in the least restrictive environment safely.

Cases are presented to a team of professionals. This may include an attorney, social worker, counselor, psychologist, nurse and geriatric specialists. A comprehensive assessment of the senior’s status reflects client/family needs, including social, medical and financial. The team, with input from the client/family, will develop a recommended plan of care; then a case manager will refer the client to appropriate services, monitor the provision of these services and evaluate the outcomes.
 
The evaluation and the clinical case management are at no cost to you. Anyone may make a referral. Participation in the Senior ICAT is voluntary and confidential.
 
Senior ICAT . . .
  • Strives to maintain the independence, safety and well-being of older adults, in the least restrictive environment, cost effectively.

  • Promotes education, cooperation and collaboration among resources.

  • Improves access to services.

  • Advocates for a coordinated multi-service delivery system for older adults, caregivers (formal & informal) which is family focused and community based. 

Consider contacting Senior ICAT if you know someone age 60 or over who has any one of the following:

  • Difficulty with daily living skills
  • Suspected abuse/neglect exploitation
  • Unreasonable fears or anxiety
  • Suspected mental health or drug/alcohol issues
  • Complicated family dynamics
  • Caregivers with suspected drug/alcohol or criminal issues
  • Homelessness
  • No running water or utilities
  • Excessive or unsanitary clutter
  • May need outside help
  • Problems with memory loss
  • Difficulty functioning, either physically or cognitively in the community
  • Isolated, alone, lack of a support system
